We are excited to have the opportunity to offer FREE Sports Physicals for any South Platte student who will be in grades 8-12 for the 2023-24 school year.
On Tuesday, May 16, Kyle Brown, PA-C, from Regional West Physicians Clinic will be offering FREE sports physicals for all students entering grades 8-12 at South Platte. The physical exam will be conducted at school during our normal school day. All students who will participate in athletic contests during the school year or participate in summer athletic camps/clinics need to have a physical examination prior to the activity. Participation in this physical examination is optional. Parents may choose to have their student’s sports physical completed by a doctor of their choice, at parent’s expense.
The forms that need completed are available in the High School Office, can be printed from the attached packet, and were sent via USPS on Thursday, 5/11/23. Please complete the forms, paying special attention to anywhere there is an *:
- History Form: Complete both sides and sign
- Medical Eligibility Form: Complete the “Shared Emergency Information” portion
- Physical Examination Form Sign the bottom of both sides
Forms need to be completed and returned to the school on or before May 16 for your students to participate in this FREE Sports Physical.
